The ECHO (End of Life Care Hub) provides a 24-hour, 7-day-a-week telephone service offering expert advice and support for referred individuals with life-limiting illnesses in Coastal West Sussex. Operated by Sussex Community NHS Foundation Trust, ECHO serves as a single point of contact for patients, carers, and healthcare professionals, ensuring seamless coordination of care. The service includes telephone triage, navigation, and signposting to appropriate services, facilitating access to necessary care and support. ECHO also records patients’ preferences and wishes in a confidential electronic care plan, which is shared with relevant healthcare providers to ensure consistent and respectful care. Operating from Lancing, ECHO collaborates closely with local hospices, the Acute Trust, and a specialist Macmillan team to deliver comprehensive end-of-life care

Adults who may have an illness or disability can access assistance with daily activities such as washing, dressing, and community involvement. The council offers assessments to determine eligibility for funded care and provides information on long-term care options through resources like the Care Guide and Carewise funding scheme. For those requiring end-of-life support, the council collaborates with the NHS to facilitate access to services like NHS Continuing Healthcare funding and palliative care options . West Sussex Urgent Community Response – Website

The West Sussex Urgent Community Response (UCR) Service, operated by Sussex Community NHS Foundation Trust, offers rapid, short-term care to adults aged 18 and over who are experiencing a health crisis or recovering from illness. Available daily from 8 am to 8 pm, the service provides urgent assessments within two hours and follow-up support within 48 hours. The multidisciplinary team includes nurses, paramedics, physiotherapists, occupational therapists, pharmacists, and health assistants, delivering care directly in patients’ homes to prevent unnecessary hospital admissions and facilitate timely discharges.
